Innovative Care

Amerimed has partnered with a number of hospitals to embed our staff in the inpatient setting enabling us to meet patients and their families before they are discharged home or discharged to post-acute facilities and then to home. This unique and progressive approach allows us to proactively follow patients through the hospital discharge process and then send professional and caring resources into the home in less than 72 hours after discharge to ensure patients are on their full road to recovery.

EXTENSIVE MEDICAL SERVICES

Amerimed practitioners offer a wide range of medical services to our patients including preventive health screenings and exams; diagnosing, treatment, testing and monitoring of chronic conditions; in-home post hospital discharge care and support; coordination of care with primary and specialist physicians for follow-up; coordination with other providers such as home health, durable medical equipment, etc.; review and reconciliation of patient prescription drugs; and other needed urgent and primary care services delivered in the home.
